1. Complete an Order Form( PDF - 210 kb - 1 page )  and/or generate a Purchase Order.
2. Please check that your Order Form/Purchase Order includes the following information:
   • Correct amount for product, including shipping and taxes.
   • Clearly identifiable purchase order number, billing address and Ship-to address
   • Contact information for each of the following persons for your site:
     • Purchaser (if different from Ship-to)
     • Main site contact
     • Ship-to contact
   Please include telephone and fax number as well as an e-mail address for each of the above persons (where applicable).

3. Fax your Order Form/Purchase Order to 1-888-349-3437 or e-mail it to sales@bridges.com.
